| Just 4 easy steps! |

2. Return your application with a copy of your driver's license and a minimum
deposit of $10.00*. The $10.00 represents a $5.00 one-time membership fee
and $5.00 is deposited into your account to become "your share of the Credit
Union".

3. Complete and return any additional applications or forms for any other Credit Union services you would like (direct deposit, ATM card, loan, etc.)

4. Complete a signed Payroll Deduction Authorization form IF you want to have money deducted from your paycheck (if you are employed with the State of
Florida) and deposited to your account each pay period.

As long as you maintain a minimum of $5.00 in your share account, you will be a member... regardless of who you are employed by!
We encourage your family members to also become members! We have Golden Year accounts for members who are 55 years of age or older and we have a Kid's Club for memebrs who are between the ages of 4 and 12.
*If you are employed with the State of Florida you can send us a Payroll Deduction Authorization and we will deduct the $10.00 from your first payroll deduction.
